SCHEDULING APPOINTMENTS

We are open Monday through Friday and have appointments available 7:30 am to 5:00 pm.

We see patients at tpur office located at 3412 W. Centre Ave, Portage, MI

Appointments may be scheduled through our office at (269) 329-5860.

YOUR APPOINTMENT

To plan your schedule, please understand that an initial or complete exam takes up to 1 1/2 hours. Both eyes are dilated for complete exams. Dilation may cause your eye to be sensitive to light and make it difficult to see things well at close range, like words on a page. It is advisable to bring sunglasses to your complete exam or to ask a friend to drive you home.

EMERGENCIES

Our physicians, along with the other ophthalmologists in Kalamazoo, share a twenty-four hour a day, seven day a week emergency service.

Should you have a problem you consider to be an emergency during our normal office hours, call our office at (269) 329-5860 and our office staff will make every effort to arrange for an immediate appointment or will refer you to the on-call physician.

After 5:00 pm, during the normal work week, and on weekends an emergency service person will respond and arrange assistance with the ophthalmologist on emergency eye service.

BILLING & INSURANCE

We are a participating provider with Medicare, Blue Cross Blue Shield and most other insurance companies. If you have questions regarding your specific plan, feel free to contact our business office at (269) 329-5870.

To better serve you, please keep us informed of any changes in your address, telephone number or insurance coverage.

  1. Kalamazoo Ophthalmology is a participating Medicare provider, therefore, we directly bill any services for Medicare patients
    • Medicare requires a $100.00 deductible at the beginning of every calendar year.
    • Medicare pays 80% of the approved amount. The remaining 20% of the approved amount is the your responsibility or a coinsurance's responsibility if that applies.
    • Not all insurance companies will cover Medicare's annual deductible. Please be aware that some supplementals also require an annual deductible. It is your responsibility to understand your policy's deductible.
  2. Medicare will not pay for glasses unless you have had cataract surgery and then will only cover the cost of a new lens for the surgical eye.
  3. Medicare and many other insurance companies will not pay for a refraction fee. This is usually an out of pocket cost to the patient.
  4. All HMO patients must have a referral from their primary care physician for an examination.
  5. If you and your insurance company have both paid for your visit, the credit balance will be refunded.

If financial problems arise, please call our billing department at (269) 329-5870. We will work with you to make arrangements that are mutually agreeable.

If surgery is planned, you and your family are invited to discuss the fee and appropriate methods of payment with our business office at the time of your pre-operative conference.

GENERAL INFORMATION

A wheelchair is available at the entrance to our office for your use if necessary. Please relate any medical problem to us when you arrive which would require special attention, so that your visit can be planned accordingly.
